Most insurance companies including Medicare, provide coverage for chiropractic care. Coverage is subject to your policy provisions. We do our best to let you know what your benefits are for care at this office. Please bring in your insurance card at your first appointment and we will call to find out what your coverage is. We will also file your charges to your insurance company for you. If there is a balance after your insurance processes your claim, a statement will be sent to you for the balance. Our statements are sent out on a monthly basis. We accept Visa, Master Card, personal checks, or cash.

| HMO's |
Teff Chiropractic is not currently a member of any HMO. However, if you have an HMO and your policy has a point of service or extended plan, there could be coverage. Charges would be subject to your off panel benefits. In most cases, off panel benefits are subject to a deductible and co-insurance. |

| Workers' Compensation Cases |
Teff Chiropractic accepts workers' compensation cases. Many employers recommend chiropractic for care of workers' compensation related injuries. All work related injuries need to be reported to your employer before your appointment. You also need to bring in the name and address of the place or company where we will file your claims. We will also need the name and telephone number of the contact person at your place of employment. |

| Personal Injury/Car Accident Cases |
We will file to your health insurance or liability insurance that provides medical benefits. If you have a lawyer and are not filing with an insurance company, we ask that you pay at the time of service unless other payment arrangements have been arranged.

| Medicare |
If you are covered by Medicare Part B, Medicare will pay 80% of the adjustment to the spine only. All other services are not covered. However, under the WI Mandate some secondary insurance companies are required to cover chiropractic care even if Medicare does not. Medicare sets our fees on an annual basis. We will file to Medicare and your secondary insurance if you have one. |

Cash Program |
If you do not have insurance, or you choose not to use your insurance, then you may pay by cash, check, Visa or Master Card at the time of service unless other arrangements are made. (Monthly payments and other payment plans can be arranged.) |

| Wellness Care |
Wellness care is where current patients seek treatment for maintenance purposes. They have regular check ups to keep their health at an optimal level. Patients are seen on a regular basis according to what the doctor recommends. It may be once per month, or once every eight weeks etc. Wellness care is not covered by insurance plans. Our current fee is $44.00 a visit and it includes your adjustments and any examination that is needed for your care. Payment is due at the time of service. |
